I consent to any pictures or videos taken of myself being used for Wild Industries/Wild Beauty Bar promotional material.
I acknowledge that I do not hold Wild Industries/Wild Beauty Bar liable for any lost or stole items, injury or accident that may occur during their time in the studio or salon space.
I acknowledge that if i am signing up for a beauty service, I am also giving my consent to receiving beauty services at Wild Industries/Wild Beauty Bar & in doing so do not hold them liable for any allergic/ adverse reactions that may occur during my time in the studio or salon space.
I consent that I am signing up for a dance or fitness course hosted by an independent contractor (Instructor) of Wild Industries/Wild Beauty Bar, and in doing so do not hold these Instructors liable for any lost or stolen items, injury or accident that may have occured during their time in the Instructors course/ workshop/class or event.
I acknowledge that I am signing up for either a dance or fitness course, booking beauty services, renting our venue privately or booking a private event package (all of which are hosted by an independent contractors) with Wild Industries/ Wild Beauty Bar of my own will. In doing so will provide all possible pertinent information to help get the best possible service. (Any health info we may need to know, preexisting allergies or injuries, possible concerns before starting the service booked.)
I hereby state that I will not cause harm of any kind to myself or others (whether it be physically, mentally, emotionally or digitally via an online platform) who are involved or related with the Wild Industries/ Wild Beauty Bar facility & brand. I consent to giving out positive & supportive vibes while booking any of our services! :)
I understand that all sales are final, non-transferable to another class/day and non refundable.
I understand & agree to all Wild Industries/Wild Beauty Bar Terms & Conditions Listed on the "Polices" page.
I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and consent to participate in this program. I agree to disclose my physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program.
